ROOT is Juniper’s pelvic floor wellness program in Manitowoc, using Winback TECAR therapy and progressive exercise. Two tracks: Strength for leaking and weakness, Relief for tightness and pain.
You mentioned it once. Someone told you it was normal – after a baby, at your age, that’s just your cycle – and you left it there.
So you manage. You know where the bathrooms are and which exercises to skip. You brace before you sneeze. You have stopped bringing up that sex hurts, or that sitting through a long meeting hurts, because you already know what you will hear.
It might have started six weeks ago or sixteen years ago. It might have started after surgery, or with perimenopause, or in your teens, or for no reason anyone has ever been able to name. You might never have been pregnant at all. Pelvic floor symptoms don’t have one cause, and they don’t come with a deadline for doing something about them.
Common does not have to be your typical, and it is not something you should have to solve alone with a YouTube video and a guess.
ROOT is our pelvic floor program at Juniper. It runs twelve sessions and comes in two tracks, because the pelvic floor fails in two opposite directions, and the fix for one makes the other worse.
Two problems that look alike and aren’t
Here is the single most useful thing to understand about pelvic floor symptoms.
A pelvic floor can be underactive: weak, slow to fire, not coordinating with your breath and your deep abdominals. That’s the floor that leaks when you cough or run, and that feels like nothing is holding. It shows up after birth, but also after abdominal or pelvic surgery, with the tissue changes of perimenopause and menopause, after years of chronic coughing or straining, and in athletes and lifters who have never been pregnant.
A pelvic floor can also be overactive: tight, guarded, gripping, never fully letting go. That one produces pain, pressure, aching that stretching doesn’t resolve, painful periods, and discomfort with sex or with sitting. It often has nothing to do with childbirth: endometriosis, chronic constipation, an old injury, years of bracing against pain, or no identifiable trigger whatsoever. For many women it started well before any pregnancy, and for some it started without one.
Both can feel like weakness from the inside. Both can cause leaking. But they need opposite work. Kegels are exactly right for the first and can make the second considerably worse, which is why generic “do your Kegels” advice fails so many women and why some people get more symptomatic the harder they work at it.
Sorting out which one you’re dealing with is the first thing we do, and it’s why ROOT starts with a phone consultation rather than a purchase.
If you have both – pain and leaking together – we start with Relief and reassess once the pain settles. Strengthening into a painful, guarded pelvic floor tends to make both problems worse. Calm it down first, then build it up.
What TECAR therapy is
Both ROOT tracks are built around Winback TECAR therapy.
TECAR – short for Transfer of Energy Capacitive and Resistive – is a radiofrequency device that moves energy through tissue and generates gentle warmth from the inside out, rather than heating you from the skin down like a hot pack. That deep warmth increases local circulation and oxygenation, helps soften scar tissue and fibrosis, and calms pain signaling in the area being treated.
It doesn’t feel like much. Most people describe it as a warm, pleasant hum. There are no needles, no shocks, and nothing internal.
There’s a growing body of research behind it in pelvic health specifically, including a 2020 randomized trial in PLoS One on capacitive-resistive radiofrequency for postpartum perineal pain. It’s also widely used in European postpartum care, which is where Winback’s protocols originate.
What TECAR does best is prepare tissue. Warm, well-perfused, non-guarded muscle responds to exercise. Cold, defensive, scarred tissue mostly resists it. That’s the logic of the whole program: use the device to open a window, then do the actual work inside that window.
ROOT: Strength
For leaking, heaviness, low back pain, and the core that won’t switch on.
The Strength track runs Winback’s Mom+ program. Winback developed it for postpartum recovery, but nothing about the mechanism is postpartum-specific – it applies to any pelvic floor that is weak and slow to recruit, whether that happened after a birth, after surgery, with hormonal changes, or over years of loading it without ever training it. It’s hands-free and takes twenty minutes.
A ground plate goes on your lower abdomen and active electrodes sit at your inner thighs. Two things then run at once. TECAR warms the tissue and increases circulation through the pelvis and deep abdominals. A gentle muscle-stimulation frequency activates the slow-twitch fibers of the pelvic floor – the endurance muscles that hold you together through a long day.
The stimulation is progressive. We start low and step the intensity up week to week, with comfort as the goal rather than intensity. There is no prize for pushing it.
That activation matters more than it sounds. A large share of women can’t reliably find their pelvic floor when asked to contract it – the wiring is intact but the signal is faint. Feeling the contraction happen makes it far easier to reproduce on your own. Awareness comes first, then strength.
Then we build on it. Each session pairs treatment with a progressive exercise plan: breath and pressure management first, then deep core coordination, then loading it – carrying, lifting, and impact work if that’s where you’re headed.
If you are recently postpartum, Winback recommends waiting at least six weeks after a vaginal birth and three months after a cesarean before starting Mom+. We hold to that, with your provider’s clearance. If your last delivery was years ago, or there was never a delivery, this doesn’t apply to you.
ROOT: Relief
For pelvic pain, tightness, period cramping, heaviness, and sluggish bowels.
The Relief track runs Winback’s Pelvic+ program – also twenty minutes, also hands-free, but doing close to the opposite job.
Plenty of the women this track is built for have never had a baby. Endometriosis, painful periods, pain with sex, tailbone pain, and chronic pelvic pain frequently start years before anyone is thinking about pregnancy, and often go a decade without a name. Some arrive with a diagnosis and some have only ever been told their labs are fine.
A ground plate sits at your low back with an active electrode over your lower abdomen. The program alternates between two settings every five minutes: one that works on releasing muscle tension, and one that helps move fluid that has pooled and stopped draining well. Unlike the Strength track, the intensity stays fixed rather than climbing.
The warmth increases blood flow, improves the pliability of scar tissue and fascia, and quiets the pain response in the area. Muscles that have been guarding for months start to let go, sometimes for the first time.
We often pair Relief sessions with PEMF, and the timing can be used deliberately – for cycle-related cramping, running the program several days before your period tends to work better than chasing the pain once it has already arrived.
Then we use the window that opens. Down-training is the actual skill: diaphragmatic breathing that lets the pelvic floor lengthen on the inhale, positional work that takes tension off the system, hip and adductor mobility, and gradual reintroduction of what you’ve been avoiding.
If you have scar tissue that’s tight, tethered, numb, or hypersensitive – from a cesarean, a hysterectomy, an appendectomy, or any abdominal surgery – this is usually where the most noticeable change happens, and often faster than people expect.
One honest limit. If you have a diagnosed condition like endometriosis, this program is not treating that condition. What it works on is the layer sitting on top of it: the muscles that have been guarding for years, the fascia that stopped gliding, the fluid that stopped moving. That layer is often responsible for more of your day-to-day discomfort than people assume, which is why the work is worth doing – but it is a different claim, and we’d rather make the accurate one.
What the program actually looks like
A phone consultation, free, about fifteen minutes. We go through a questionnaire and talk it through. We’re working out which track fits, whether anything rules out TECAR for you, and – honestly – whether we’re the right place for you at all. Nothing gets scheduled on that call if the answer is no.
Paperwork and baseline questionnaires. Before your first visit we send a consent form and the questionnaires for your track, so you can fill them out at home rather than in a waiting room. Strength clients complete the ICIQ, PFDI-20, and PFIQ-7 – the same validated tools used in pelvic health research. Relief clients complete a pain-focused equivalent.
First appointment, forty-five minutes. Longer, because we’re setting up – your first treatment, a walk-through of the home exercises for your track, and time to ask everything you didn’t think to ask on the phone. You’ll leave with a bladder diary or a symptom journal, depending on your track.
Follow-up sessions, thirty minutes, two or three times a week for four to six weeks. Treatment plus the exercise progression that goes with it. These run in the Nest – blanket, weighted blanket, eye mask, music if you want it. Twenty minutes where nobody needs anything from you is, for a lot of our clients, not a small part of the appeal.
Final appointment. The same questionnaires come to you again beforehand. We sit down with both sets side by side, collect your diary and journal, and compare. Pelvic floor progress is easy to lose track of, because it happens gradually and you adapt to it as it goes. Numbers keep you honest, and they keep us honest.
What ROOT is not
This part matters, and we’d rather be direct about it than have you find out later.
ROOT is a wellness program. It is not pelvic floor physical therapy, and it is entirely external. We do not perform internal pelvic floor assessment or internal treatment. Nobody at Juniper is going to examine you internally, because that isn’t our scope.
It also is not treatment for an underlying medical condition, and it does not replace the gynecologist, urologist, or specialist managing one. For a lot of women, external work plus good coaching plus the right progression is genuinely enough. For some, it isn’t. If your presentation calls for internal assessment – significant prolapse symptoms, pain that needs to be mapped internally, anything that isn’t sorting out the way it should – we will tell you and refer you to a pelvic health specialist.
We’re not trying to be a substitute for that care. We’re glad to run ROOT alongside it, and referring to you isn’t us giving up on you. It’s the point of the screening call.
Some conditions rule out TECAR therapy entirely – active pregnancy and certain implanted electronic devices among them. We go through the full list before you commit to anything.
